Co-developing a model to support better journeys for young people exiting custody and out-of-home-care in South Australia

Support systems for young people leaving out-of-home-care and custody are complex, disjointed, difficult to access and often offer generic, rather than tailored, wrap-around approaches. Accessing support in this context is challenging and can increase the risk of homelessness and criminalisation. This presentation will explore the Constellation Project’s ‘Systems Navigator’ project, offering key insights and learnings acquired throughout co-design the journey. This project aims to explore whether a ‘systems navigator’ model could support young people leaving out-of-home care and custody to navigate the complexities of these systems and prevent homelessness. With a commitment to working differently, we have prioritised the voices of lived experience and First Nations peoples at each level of the design and development and recognise securing housing outcomes as inherently linked to all other aspects of wellbeing and life. Developed since April 2021, through multiple iterations of co-design, the model is now in its final stages and ready for further co-design with young people with lived experience of incarceration. This will be followed by testing and refinement with First Nations communities and people with professional experience in the child protection systems, youth justice system, and relevant services. This phase is anticipated to be completed by June 2024, after which a pilot proposal will be developed.
